Fixing dog/people agression

I have a 2 yr old male Maltese who is aggressive towards strange people and dogs. He lunges, has biten and doesn't listen very well outside. I'm reading "Click to Calm" and understand all the concepts in this book, but I am just unsure where to begin. My dog knows and does commands inside most of the time, but outside he doesnt listen very well at all. Please help
